Job Requirements:

Must have a minimum of a Bachelors of Science degree in Nursing or Health Care Administration, with Masters Degree preferred. Must have a minimum of five years of progressively responsible managerial experience, with baseline knowledge of case management standards, and an understanding of utilization management principles. Current/Active State of FL RN License required - primary source online verification required directly from State of Florida Licensing database. Certification in Case Management preferred.

Job Description:

The Director has the responsibility to establish and carry out the departmental policies and procedures appropriate to the framework to support the services provided. Support the policies and procedures, goals and objectives of the Case Management Services Department and the Health System; including but not limited to the risk management programs, safety, environment and infection control standards. Directs the case management supervisors of the department and through these individuals supervise all associates working within Case Management. Works with other department directors throughout Martin Medical Center, Martin Hospital South and Tradition Medical Center to assure that the post acute care needs of the patients are being met. Ensures that Case Management is responsive to the needs of the patients, their representatives and the community. Develops and implements policies, procedures, and protocols for the department under the direction of and in compliance with corporate standards, local, State and Federal Regulations and considering standards of practice of Case Management. Develop and monitor Center for Medicare and Medicaid Recovery Audit Contract (RAC) demonstration project. Lead teams that develop and monitor programs such as MS-DRG implementation, and Center for Medicare and Medicaid Core Measures Quality Indicators. Collaborates with nursing leadership for healthcare team design modification, process changes and implementation. Fosters good communication and relationships with other corporate departments in addition to physicians, associates, patients and their families and our referral sources. Develops and manages the operational and capital equipment budget for the department, assuring appropriate resource allocation and cost containing measures, whenever possible. Participates in the corporate quality improvement programs and assists in the development of a department specific performance improvement plan. Responsible for departmental associate functions such as hiring, supervises associates employed by the department, providing counseling when necessary in accordance with corporate standards. Facilitates growth and development of the case management program consistent with department goals and Health System mission through networking, quality management and other activities as needed. Responds to patient and family concerns within a reasonable time frame. Conducts and directs the preparation of department reports with periodic communication for senior leadership as indicated. Consults with medical, nursing, administrative staff on a regular basis. Works collaboratively with Utilization Management department and attends the UM Committee meetings. Participates in hospital and medical staff committee meetings as required. Attends and serves on professional and civic service organizations as hospital representative. Maintains professional growth and development through seminars, workshops, and professional affiliations to keep abreast of latest trends in case management. Maintains annual mandatory education requirements specific to position as mandated by Martin Health System.
