Master's Degree in Nursing required. Current RN license with ability to practice in the State of Iowa. Minimum of 5-7 years of related experience including five years in a managment/leadership role. Demonstrates specific knowledge related to software programs such as Cerner/PowerChart, HealthQuest, Milliman Guidelines, CDI software and other appropriate software programs used in the course of case management processes. Experience with computer applications such as Microsoft Office Suite (Word, Excel, PowerPoint, GroupWise), HealthStream, Intranet; and use of reporting tools.

Job Description:

The Director of Case Management and Social Services is a member of the Mercy Medical Center leadership team. Provides leadership in planning, organizing and directing the operational and strategic functions of the Case Management & Social Services departments, and ensures excellent coordination of care, effective transitions across the healthcare continuum, optimal utilization of resources in care delivery, and appropriate clinical documentation consistent with current regulations, guidelines and future trends. The director develops an effective infrastructure, and ensures inclusive decision-making, an evidence-based approach, and an effective leadership style in achieving excellence in areas of responsibility. Leads engaged team members in advancing the strategies and goals of the organization, and supports a culture, which promotes collaboration, ongoing learning and a results-orientation. Embraces and models the Magnet standards of excellence, and cultivates this framework through her/his leadership. Works collaboratively with all stakeholders and creates/sustains a service that provides innovative and effective services. Promotes staff member accountability for their own practice, and recruits and retains employees with the appropriate values, experience, education and background to meet organizational/service expectations. Leads the development and ongoing refinement of efficient and exemplar processes; and achieves outcomes that meet or exceed expectations/targets in all areas of responsibility.
