Job Requirements:

Ideal candidate (s) will have at least 3+ years Case Management experience.
Utilization Review experience
Third party payer knowledge preferred.
Experience with Intraqual preferred
Hospital - inpatient experience preferred
Coordination of care in an inpatient or home setting
BSN or ASN currently enrolled in BSN program
 Ability to effectively use specialized computer based systems for the gathering, reporting, and analysis of clinical data. 
Excellent organizational and communications skills required.

UMass Memorial has exciting Full Time Day opportunities in the Care Coordination Department in Worcester for RN Case Managers with at least 3+ years of experience in Utilization Review and Discharge Planning.

Position responsibilities include the following:
Assess patient’s physical, functional, and psycho-social needs through the use of screening tools, direct patient and family communication, medical record review, and discussions with other care givers providing services to the patient.
Identify complex patients in need of intensive case management based on established clinical and financial criteria.
Perform admission and concurrent reviews as assigned. 
Perform inpatient reviews in accordance with payer requirements utilizing clinical criteria sets. 
Educate practitioners on the level of care and documentation requirements.
Facilitate the care of patients throughout the continuum of care.
Work as a team member to support /collaborate with Care Coordination Team for monitoring level of care, and for identification of trends in clinical practices and opportunities for improvement.
