Job Description:

Full time RN position in the nationally recognized Elder Service Plan PACE (Program for All inclusive Care to the Elderly) program which provides integrated medical, rehabilitation, case management, adult day health and home care services.
 
In this full time primary nursing role, the nurse will function as a member of the inter-disciplinary team in the planning, coordinating and provision of care to participants in their homes and in the PACE center. Requires current MA RN License with minimum of 1 year of experience in home health, ambulatory, hospital or sub-acute setting preferred. Also requires minimum of one year experience working with a frail or elderly population. BSN preferred and Bi-lingual in Spanish is a plus. Prior experience in interdisciplinary team setting and home care experience is preferred. Schedule: Monday - Friday, 8:00 AM - 5:00 PM, and will include holiday and on-call rotation. Requires your own transportation.
